Sponsorship Pledge Form
Your sponsorship provides great visibility for your service, program, agency or facility and is an expression of your commitment to your staff and quality care in Iowa. Sponsors are acknowledged on printed conference and promotional materials, news releases, in the conference script and signage. Sponsorships will be recognized in the following levels:
Sponsorship Levels

Co-sponsor $5,000 and above

Five Star $3,000 to $4,999

Four Star $1,000 to $2,999
- Three Star $100 to $999
Options for Sponsorship
- Keynote Speaker $4,000 (includes travel expenses)
- Reception & Awards $7,000
- Opening Speaker $1,000
- General Session Speaker $2,000 (includes travel expenses)
- Closing Keynote Speaker $2,500 (includes travel expenses)
- Conference Scholarship Fund $250 per person (2 day conference)
